The Family Partnership Model grew from serving three (3) families in 2007 to nearly 300 in 2023. Over a thousand families maintained their housing, built transformative connections with their mentors long after exiting the program, accessed living-wage career pathways, received financial coaching, and acquired affordable childcare to build and sustain overall family well-being. In 2024, we achieved the following outcomes:

• 96% of participating families maintained their housing.

• 100% of school-aged children were enrolled in school.

• 94.5% of adults and 86.5% of children visited with a Primary Care Physician to examine their wellness.

• 78% of family members who have a mental health need and/or diagnosis were referred to therapeutic services.

• 82.5% of eligible families who received a benefits assessment filed their taxes.

• 66% of households had at least one able-bodied adult who was employed, enrolled in school, or enrolled in a workforce development program.

In 2023, we piloted the Benefit Navigator, a one-stop-shop online tool for case managers, individuals, and families to help them understand, access, and navigate the complex public benefits and tax credit landscape– quickly, confidently, and in one place. The initial pilot, which involved 10 social services agencies, and a robust third-party evaluation, revealed the following inspiring results:

• 44% more benefits/credits identified than by the control group not using tool.

• 24% average increase in earned income in 6 months as compared to a 16% decrease in earned income by control group (attributed to clients’ ability to see impact of income on their benefits).

• Case workers and their managers thanked us for creating a tool that actually helped them do their jobs.

• In 7 months, the Navigator identified over $31 million in additional eligible benefits and tax credits – an average of $15,770 per person.

As of January 15, 2025, Benefit Navigator pilots have expanded to:

• 500+ case managers trained users across 55 Los Angeles County non-profit, government and healthcare service organizations.

• 11,000+ clients have identified $125+ million dollars in additional benefits and refundable tax credits!

• Plus, the transparency generated by the Navigator has helped many of these clients decide to work or work more, because they are confident that they are not going to lose the public benefits they need before they are ready.
