Imagine LA is a nonprofit organization operating throughout LA County to interrupt the cycle of family homelessness and poverty.
The Family Partnership Model
Imagine LA’s Family Partnership Model includes the following:
• Intensive Case Management (Imagine C.A.R.E) Upon enrollment with Imagine LA, you will meet your Family Team Manager (FTM) who will guide you through all steps of the program. Your FTM will conduct a psychosocial assessment with your family to understand how best to support you and will partner with you to build your Case Plan, establishing the highest priority needs and goals you would like to tackle during your work with us. Typically, our families pursue growth in the areas of employment, education, financial literacy and stability, physical & mental health, resolving legal challenges, and more. You will meet monthly with your FTM to discuss how your family is doing, how your goals are progressing, and work together to identify and remove barriers to those goals. Your FTM will provide linkage to needed resources to ensure your family feels cared for and connected throughout the Imagine LA experience.
• Economic Mobility Imagine LA experience. Through the Economic Mobility Program, Imagine LA proactively equips its families to reach housing stability and long-term well-being by providing them with connections to reliable financial education and coaching, comprehensive career training, and safe and affordable childcare as well as, centralized information to effectively navigate the social benefits. All these services are free and offered by our long-standing partners in each of the fields through a thorough referral process.
• Family Mentorship Families enrolled in Imagine C.A.R.E can begin with Mentorship when they feel prepared to welcome new, transformation relationships into their lives. The purpose of your mentor team is to maximize avenues of support and collaboration for your family by providing one-on-one mentorship for every family member aged 5 and above. The benefit of mentorship is that each family member receives highly relational, individualized support from someone dedicated specifically to them and their goals. The goal is for each family member to see their mentor in person twice per month. Your FTM will lead a quarterly team meeting every 3 months with your whole family & all mentors present to make sure everyone is on the same page with goals and priorities for the upcoming few months. You will have the opportunity to work with your mentor team for a period of 12-18 months.
Potential Risks
Imagine LA cannot guarantee that every mentor match will be successful. Please communicate clearly with your FTM as your relationship with your mentor team progresses—if an issue arises that cannot be resolved, Imagine LA will commit to re-matching a new mentor to a family member one time, dependent on availability of volunteers. Imagine LA also cannot guarantee outcomes for our participants. The greatest determining factor in you achieving the goals set out on your case plan is your personal commitment to fully engage with the program, communicate your needs and wants clearly, and work through barriers collaboratively with your FTM and mentor team.
Limitations of Services
Please note that Imagine LA is not a mental health service provider, and as such our FTMs will not be providing therapy services to your family. We are committed to assisting your family in locating a mental health provider you feel comfortable working with and will look forward to collaborating with other service providers involved in your life.
Costs
Imagine LA is a nonprofit service provider and there is no cost to be involved in any of our programming.
Injuries or accidents
Imagine LA will always do our best to ensure that any event we run (including but not limited to Love Yourself, Celebration of Family, Back to School, Winter Wonderland, and any and all in-person meetings, convenings, or gatherings) is safe for all attendees. However, accidents happen. In the event of an injury or accident at or related to an Imagine LA-hosted event, you hereby waive and release Imagine LA from any and all responsibility for any claims, damages, and expenses that may arise directly or indirectly, however caused, from your or your family’s participation in the event/activity.
Confidentiality
Imagine LA is committed to abiding by ethical practices surrounding confidentiality and privilege. Information about you and your family will not be shared with any outside party without your written consent via a release of information. We ask that you provide releases of information for the party that referred you to Imagine LA, as well as for any other service providers you are connected with who could provide helpful information that would equip us to support you well in pursuit of your goals (mental health providers, school officials, healthcare & housing providers, etc.)
Limits to confidentiality
Please note that Imagine LA staff and volunteers are mandated reporters. We are legally required to report the following to the appropriate agencies:
-Knowledge or suspicion that a minor child (under age 18) is experiencing abuse or neglect
-Knowledge or suspicion that an older adult (over age 65) or dependent adult (unable to fully care for themselves) is experiencing abuse or neglect
-Knowledge of ongoing or current physical harm or imminent risk of harm to any individual, whether part of your family unit or not
-Clear and imminent risk that an individual is a danger to self or others
-If at any point Imagine LA is court-ordered by subpoena to release records to the court, we must comply (this would be discussed with the head of household prior to any documents being released)
Respectful Relationships
The Imagine LA program is all about building mutually trusting & supportive relationships in which everyone involved can thrive. All Imagine LA Participants (families, mentors, staff) are expected to be considerate and respectful of the rights and property of others. By signing below, you agree to refrain from abusive language, bullying or threatening behavior, possessing or being under the influence of drugs or alcohol during Imagine LA activities, or possession of weapons or firearms during Imagine LA activities. Participation in Imagine LA's program is subject to the observance of the organization's procedures as outlined here.
