Replacing Vital Records:

Replacing records lost in fire is free. Call 800-201-8999 and select Option 1, followed by Option 2, or email RRCCFireAssistance@rrcc.lacounty.gov for assistance. See link for more info and onsite locations.

Replace Lost Social Security Cards
Replacing a social security card is free. To do so, use the provided website and fill out the "Answer a few questions", or call +1 800-772-1213.

To Replace Driver's License:
- Use Online link $36 fee.
- In-person, mention lost in fire - fee waived.

To replace a passport, you must first report it lost or stolen, which can be done via online form at the Department of State.
Information needed (Be sure to follow instructions provided very carefully).
- Full name
- D.O.B.
- Social Security Number
- Height
- Occupation
- Address
- Passport Number and date issued. YOU CAN LEAVE THIS BLANK IF YOU DO NOT KNOW IT.
The form filler will then generate for you DS-64 and DS-11 forms.
This process will require you to PRINT OUT and MAIL forms to the government, or in the case of the DS-11 will require you to appear IN-PERSON before signing and submitting it there.
Processing times can vary, but usually are around 4-6 weeks.
